TERMS AND CONDITIONS
All deposits and subsequent payments are non-refundable. Please review the cancellation policy before making your reservation.
VIP EVENT LIST: VIP events are limited to 150 guests only. VIP Event List is updated on a 1st come, 1st served basis. By submitting an early deposit, a traveler will increase their odds of being placed on the VIP Event List. Once all 150 spots are taken, a wait list will be created. Guests who are placed on the VIP Event List, will receive confirmation of being on the list AFTER successfully completing online registration for all guests sharing the same stateroom (a separate email will be sent with steps to register all guests, we recommend you complete this within 24 hrs of submitting your deposit). Remaining on the VIP Event List is contingent upon adherence to the payment plan requirements as outlined below. Failure to make payments as outlined, will result in forfeiture of VIP event access. If a spot is forfeited, guests on the wait list will be moved to the VIP Event List until all 150 spots are filled. Guests whose VIP event access has been forfeited will be placed on the wait list once payments are received. On time (or early) payments are essential to remain on the VIP Event List.
GUARANTEED PREMIUM ROOM LOCATION: Guaranteed Premium Room Location is limited to the 1st 50 staterooms booked. For the purposes of this promotion, a Premium Room Location is defined as any room located mid-ship on deck 5 or any room on decks 6 (Upper deck), 7 (Empress deck) or 11 (Verandah deck). Guests may request a specific deck, however, all room locations are based upon availability at time of assignment. Exact staterooms will be assigned AFTER the 1st $100 per person payment has been received. This payment is due on or before April 27, 2018, however, early payments are accepted to have room assigned sooner. Keeping a Guaranteed Premium Room Location is contingent upon adherence to the payment plan requirements. Failure to make payments as outlined below, will result in forfeiture of VIP status and Guaranteed Premium Room Location. Guests whose VIP status has been forfeited will lose their Guaranteed Premium Room Location and will be placed on the wait list once payments are received. On time (or early) payments are essential to keeping Guaranteed Premium Room Location.
PAYMENT PLAN REQUIREMENTS: Click here to request a payment plan. Payment plan is required after initial deposit. All payments, including deposits are non-refundable. A portion of your payments made after initial deposit may be transferable to another trip booked with Mayfield Elite Travel if the request is received prior to the FINAL PAYMENT due date. Missing or being late on a payment, may result in your reservation being suspended and/or cancelled. If this happens, you may forfeit all discounts, promotions or incentives associated with the original reservation. Reservations that are suspended or cancelled will need to have all missed payments made to be re-booked. Click here to request a payment plan.
LATE FEES: Payment reminders will be sent to all guests booked. Missed payments may result in forfeiture discounts, promotions or incentives associated with the original reservation and run the risk of reservation being cancelled. Reservations cancelled due to non-payment are not eligible for a refund and are non-transferable. Final payments not received by November 16, 2018 will be assessed a one-time $55 late fee. Final payments not received by December 3, 2018 will be assessed a $110 late fee. Final payments not received by December 14, 2018 will result in cancellation without refund.
DEPOSITS: All deposits and subsequent payments are non-refundable. Please review the cancellation policy before making your reservation.
PASSPORTS: Passports are NOT required, but are strongly recommended. If not using a passport, an original or legible copy of your birth certificate and original non-expired government issued photo ID is required to board the ship. Click here to learn more about required travel documents.
CANCELLATION POLICY: All deposits and subsequent payments are non-refundable. If you cancel before November 16, 2018, you may be eligible to transfer your payments (not including full deposit of $100-$300 per person) to another trip you book with Mayfield Elite Travel within 12 months of your original booking date. Your cancellation must be received in writing prior to November 16, 2018. After this date, please refer to the terms & conditions of your travel insurance policy. Please remember you must keep your payments current to avoid forfeiting any discounts, promotions, incentives or risking complete cancellation without refund.
NAME CHANGES: Name changes are allowed for free up until your room is assigned only. Once your room has been assigned, a name change fee will apply. See below for name change fees. If the new traveler is replacing someone who has missed payments, the new traveler will be required to make those payments at the time he/she is added. If the missed payments have caused the cabin rate to be forfeited, the current rates will apply at the time the new payments are received. In the event that we sell out of space prior to the payment being received by the new traveler, the initial deposit and all other payments received will be forfeited.
Please note, complete names changes of everyone on the reservation are not allowed. At least 1 original traveler who booked in the room must remain on the reservation at all times, otherwise this is considered a cancellation, not a name change.

SERVICE FEES: Mayfield Elite Travel reserves the right to apply service fees for services requested in excess of basic service to your reservation. These requests may include, but are not limited to - refund requests for over payments, multiple name changes, multiple trip changes, adding special service items to a reservation and more. Mayfield Elite Travel's service fees start at $10 & go up depending on the request(s). Service fees must be paid prior to the service being rendered.
ONLINE CHECK IN: All travelers are required to complete his/her online check-in prior to the cruise. FAILURE TO COMPLETE ONLINE CHECK MAY RESULT IN YOUR BEING DENIED BOARDING OF THE SHIP. TRAVEL DOCUMENTS: Currently, only proof of citizenship (birth certificate) and a government issued photo ID is required to cruise to the ports on our schedule. However, we HIGHLY RECOMMENDED you take a PASSPORT or a US PASSPORT CARD on your cruise. Without a passport, you will not be permitted to fly into or depart some of the places visited by air if necessary, even in case of an emergency. Please note: passports are required to be valid for 6 months from your return date. TRAVELING WITH MINORS: The minimum age for cruising is 6 months. Children under 18 years of age not traveling with both parents or legal guardian(s) may need to present a notarized letter from the absent parent/guardian granting permission to travel outside the United States and provide emergency medical care if necessary. BRINGING BEVERAGES ON BOARD: Bottled beverages of any kind (including bottled water) are STRICTLY PROHIBITED from being brought onto a Carnival Cruise, with the exception of wine or champagne. Currently, guests 21+ are permitted to bring one (1) standard-sized bottle of wine or champagne on board per person in their CARRY-ON BAG. Carnival Cruise Lines reserves the right to update, change or modify the liquor policy at ANY TIME. Mayfield Elite Travel is not responsible for any changes to the beverage policy. DINNER DRESS CODE: ** MEN ** - Please be advised that some cruise restaurants may require long pants, collared shirt, & closed-toe shoes. Please pack accordingly.
ADDITIONAL TERMS: Carnival reserves the right to asses a fuel surcharge if a rise in the market price of oil necessitates a supplement. There is no fuel surcharge currently in place. Taxes, fuel surcharges, port charges and all fees are subject to change.

By attending this event, you agree to not hold Mayfield Elite Travel (or any Mayfield Elite Travel entity), responsible for any errors or omissions relating to this event. Mayfield Elite Travel reserves the right to use all client images or likeness in connection with still photography or recorded video. Attendance at any Mayfield Elite Travel event gives consent to be photographed for use in print or electronic publicity on social media sites. To ensure the privacy of individuals and children, images will NOT be identified using full names or personal identifying information if we receive documentation from the photographed subject, parent or legal guardian, declining to be photographed. If you do not want us to use a photo or video of you, please send all request to firstname.lastname@example.org at least 24 hrs. prior to the event.
